Menopause Hormone Shortage Sits Outside Health Canada Tier 3 List
Canada's current supply challenges affecting estrogen patches and Premarin Vaginal Cream have not been elevated to Health Canada's Tier 3 designation, the most serious shortage classification, according to the Canadian Pharmacists Association's active shortages register.

The Canadian Pharmacists Association lists menopausal hormone therapy among the current shortages it is actively tracking, noting supply challenges affecting some estrogen products in Canada, including patches and Premarin Vaginal Cream. CPhA states that Health Canada is in contact with manufacturers and is closely monitoring the supply situation, and that alternative products remain available.
CPhA attributes the intermittent shortages to increased demand for menopausal hormone therapy products combined with manufacturer disruptions. The association published a dated clinical resource, the Menopausal Hormone Therapy Shortage document, on March 13, 2026.
How the tier system works
Health Canada's Health Product Shortages Directorate convenes a Tier Assignment Committee when a shortage has the potential to pose a significant risk to patients and the health-care system. That committee determines whether a shortage should be classified Tier 3, the most serious level, and shortages given that designation are posted on Health Canada's Tier 3 shortages website.
For comparison, Health Canada declared a Tier 3 shortage of cyclophosphamide for injection on February 20, 2026 after all three Canadian-market manufacturers reported constrained supply. Ipratropium nebules have also been designated Tier 3. The menopause hormone therapy entry on CPhA's page carries no equivalent Tier 3 declaration.
Scope of estradiol supply disruptions
Drug Shortages Canada records that since 2017 there have been 92 shortages and four discontinuations of products containing estradiol, spanning transdermal patches, pills, vaginal rings, topical gels and creams. Health Canada, in a written statement cited by CBC, indicated that hormone replacement drugs were generally available in Canada with the exception of estrogen/progestin patches, with an estimated end date for that shortage of July or August.
Pharmacists.ca notes that at any given time there are between 1,500 and 2,000 active drug shortages in Canada. Health Canada attributes shortages to manufacturing problems or delays, difficulty obtaining raw materials, sudden increases in demand, discontinuations, natural disasters, and pandemics.
Federal mitigation tools
Health Canada lists options it may use when a shortage requires national-level intervention, including using regulatory powers to allow drugs labelled and approved for other markets to be imported into Canada, and fast-tracking regulatory reviews. The department may also extend a drug's shelf life if supporting data are suitable.
The Multi-Stakeholder Steering Committee on Drug Shortages, established in 2012, brings together industry associations, federal, provincial and territorial governments, and health-care professional associations. Health Canada co-chairs the committee along with a rotating provincial or territorial co-chair.
